HOW TO ADD YOUR SHOW
TO THE FADS SHOW LIST

LONG FORM FOR
NEW OR DIFFERENT SHOWS


USE OF THE FORM BELOW:
  1. Use this form if you want to list a show for the first time or a show whose information is different for previous shows (different secretary, different manager, etc.).  If you have problems with the form, please contact Leslie at [email protected].
  2. When you determine that you want to put your show(s) on the FADS Show List, use the form below to provide the information to the FADS web master.  To ensure riders have show information in a timely manner, please submit the form ASAP. 

    LONG FADS SHOW FORM

    Place your cursor over each field and instructions will pop up.
    Enter the name of the person submitting this form.
    Enter the email address of the person submitting this form.
    Enter the telephone number of the person submitting this form.

    Enter the name of the farm and the address.
    Enter the farm's web site URL or none if the farm does not have a web site.

    Enter the day and date of the show. For example, Saturday, May 5, 2015.
    Enter the day the show opens.
    Enter the date the show closes. Entries can be received until midnight of this date.

    Enter the name of the judge. Enter TBD if not yet determined.
    Enter the status of the judge. For example: none, L, r, R, S, etc.

    Enter the name of the show secretary. All FADS online entry emails will go to this person.
    Enter the email address of the show secretary. All FADS online entries will go to this email address.
    Enter the show secretary's phone number.

    Enter the name of the show manager if this is different from the show secretary.
    Enter the show manager's email address if from the show secretary.
    Enter the show manager's phone number if different from the show secretary.

    Enter the fee per ride. If you have a variety of fees, indicate the prize list.
    Enter a link to your Prize List from your web site or indicate that the file will be emailed to [email protected].
    Hold Harmless Agreement must include volunteers as a covered entity. If a separate file from your prize list, provide the URL or indicate that the file will be emailed to [email protected]. If part of your prize list, indicate this in the answer box.
    Click on each box that applies to your show.
    Click one circle (YES or NO). If you plan to use FADS ribbons, which are available at cost if requested with lead time, email [email protected] with number of ribbons needed. Ribbons must be paid for when received. If you do not use all the ribbons, you may return unused ribbons for a full refund.
